Casino Discount Card Order Form

We now have two methods of ordering and payment. You can order your Casino Discount Cards by making an online payment with your credit card via PayPal. Your order will be received and fulfilled once PayPal confirms your payment.

The second option is to submit your order using the form below, and then mail a check or money order as payment to the address listed below. When submitting your order with the form below your payment will still need to be mailed prior to a card being sent to you.
